COMPLETE PERSONAL SUPPORT
More than a Service - Someone who genuinely gets to know you
One trusted person who knows the whole picture, understands what matters and
quietly cares for the details exactly as you would like.
Personal, not impersonal
Care that feels personal - because it is.
Leaving Well is intentionally personal.
You are not allocated a rotating support worker, passed between departments or required to explain your story repeatedly. When you engage Leaving Well, you work directly with me.
I take time to understand who you are, how your family works, what matters to you and how you would like things managed. I become the familiar person who remembers the details, notices when something has changed and knows what needs to happen next.
Clients often experience my presence as something closer to having an informed daughter or trusted friend beside them, someone who cares personally, will speak honestly and is prepared to step forward when things need to be handled.
I also bring the knowledge, professional boundaries and practical capability to advocate, coordinate and remain steady in circumstances that may feel overwhelming to those closest to you.
Whole-picture coordination
Let someone else hold the details for a while.
As circumstances change, families can find themselves coordinating appointments, providers, information, paperwork, household needs, family communication, future wishes and increasingly urgent decisions.
Within the scope we agree upon, I can keep track of information, attend appointments, prepare questions, follow up agreed actions, communicate with family and professionals, organise practical arrangements and ensure your wishes remain visible throughout.
I can work quietly in the background or take a more active coordinating role. You decide what you would like to retain, what you want help with and what you would prefer me to oversee.
You remain involved in every choice. My role is to make the practical parts lighter and help your wishes become the way things are actually done.
Personal management may include
· A detailed understanding of your priorities, wishes, routines, relationships and the people involved
· Appointment scheduling, attendance, notes, question preparation and follow-up
· Coordination across medical, care, allied-health, household and funeral professionals
· A clear record of decisions, contacts, agreed actions and changing needs
· Sensitive family communication, meetings and updates
· Household arrangements, visitors, meals, respite and practical support
· Meaningful experiences, living wakes, legacy projects and important conversations
· Advance planning, personal documents, funeral wishes and Swedish death cleaning
· Responsive support when circumstances change
· Continuity through dying, after-death care, funerals and bereavement
Family support
Support for you, and for the people who love you.
My primary commitment is always to the person at the centre of the experience. But illness, dying and death affect an entire family.
I help family members understand what is happening, communicate when conversations feel difficult and find practical ways to contribute. I can keep people informed, reduce confusion and relieve some of the pressure that often falls upon one exhausted relative.
I am not there to replace family. I am there to help everyone feel more capable, connected and supported, so loved ones can spend less time coordinating everything and more time simply being a partner, child, sibling or friend.
Bespoke engagement
Entirely shaped around you
Support may be arranged for a defined period, an ongoing personal-management relationship or comprehensive involvement through a significant transition.
I may work quietly in the background, coordinate the family and professional team or provide direct personal presence.
There is no standard package you must fit.
Promise: Discreet. Responsive. Exceptionally personal. One trusted point of contact, with support that expands or reduces as life requires.
